    Staywell Health Plan 2019 Healthy Behaviors Annual Evaluation Staywell Health Plan (Staywell) believes healthy behaviors lead to good medical care. Managing health needs, improving habits that affect health and taking advantage of preventative screenings are important. Staywell members making healthy choices are rewarded when completing a healthy behavior. Our program includes rewards for these activities:  Stop smoking/tobacco program  Weight loss program  Alcohol and substance abuse recovery  Well-child and adolescent visits  Prenatal and postpartum care  Women’s screenings  Diabetes management  Adult health screening The Healthy Behaviors Program runs on a calendar year. The evaluation period of this report is July 1, 2018 – June 30, 2019. The table below shows how many Staywell members earned rewards during the evaluation period. Our Provider Relations Representatives and Quality Practice Advisors educate providers on each of the available programs. Providers are encouraged to refer members to the programs. Our Care Management staff and Patient Care Advocates promote the programs and refer members as appropriate. They work with members to schedule appointments to address care gaps (well- child visits, screenings, etc.). Additionally, our Customer Service Representatives educate members about care gaps during inbound calls and will offer assistance with scheduling appointments and arranging transportation. ***** Dental Health CheckUp Program was discontinued effective Q4 2018. ***** Lead Screening Program was discontinued effective Q1 2019. ***** Teen Pregnancy Prevention Program was discontinued effective Q3 2018.
